Police have identified an Innisfil man killed Monday morning in a flying tire incident on an Ontario highway.

Ontario Provincial Police said 36-year-old David Tompkins died after his minivan was struck by a tire that came off a sport utility vehicle on Highway 400, near Bayfield Street, in Barrie.

Tompkins was with his wife and daughter when the tire caved in part of the van's roof and windshield.

Tompkins' wife and daughter, and the driver of the other vehicle were not injured.

Tompkins is the second person to be killed in a tire mishap in southern Ontario in the last seven months.

In December, a 54-year-old Hamilton woman was killed when a tire came off a tractor-trailer and struck her car on the QEW near the Burlington Skyway.
